True Spermatic Cord Lipoma Presenting as a Recurrent Inguinal Mass after Inguinal Hernia Repair: A Case Report

|
True spermatic cord lipoma is rare and has a challenging diagnosis; it is often misdiagnosed as an inguinal hernia. Here we report the case of a 60-year-old male patient for whom complete excision of true spermatic cord lipoma, mimicking recurrent inguinal hernia, was performed. The patient had undergone left inguinal hernia repair 6 years ago. At that time, minimal hernia protrusion was identified with a considerable amount of fatty tissue around the spermatic cord. A tension-free hernioplasty using mesh was performed, and most of the fatty tissue around the spermatic cord was removed. Four and half years after the operation, the patient felt a palpable, non-reducible mass in the left inguinal area. The mass was enlarged during observation. Second operation was performed to excise a 10¡¿20-cm sized true spermatic cord lipoma. The patient was discharged uneventfully. True spermatic cord lipoma should be considered in cases of unusual inguinal mass mimicking hernia.
